Although the site is primarily aimed at providing professional consultations to dentists and hygienists, a section for the public is provided where they can get advice on their dental or periodontal problems.

This form should be filled out in as much detail as possible to ensure that we have as much information as possible to give you the advice and help that you need.

The form must then be returned to us electronically or physically for processing. Radiographs and/or photographs should be included if available (in gif or jpeg format only if sent electronically).
