Periodontal-net applies the principle
of strict client confidentiality at all times.
Periodontal-net provides a consultant's report based on information provided
by the client and cannot accept responsibility for any false or inaccurate
data thus supplied.
Periodontal-net will not knowingly insert inaccurate information into an assessment
at the request of a client.
Periodontal-net cannot accept responsibility for inaccurate details or mistakes
once the final work has been received by the client and has been in their
hands for seven days. Any corrections required after that period will be charged
at the rate for editing copy.
Periodontal-net will take every precaution to safeguard client details, but
Periodontal-net cannot accept responsibility for third party interception
of data during transit (on-line ordering, e-mail, fax, post). We strongly
advise clients to select the most secure method of submitting their confidential
data.
Periodontal-net issue each client with an ticket number.
Periodontal-net will not accept instructions from clients unless the ticjket
number is attached to each instruction.
Periodontal-net cannot be held responsible for any technical difficulties
associated with the operation of the Internet (World-Wide-Web), which is entirely
outside its control. Periodontal-net will not be held responsible for e-mail
addresses provided by clients that are incorrect.
Periodontal-net will not be held responsible for any assessment request sent
by e-mail that may result in any harm, damage or any loss of important data
to any computer systems.
Periodontal-net ensures that all data transmitted and received is checked
by anti-virus software which is regularly updated. If a virus is found in
e-mail from a client, the client will be notified and no further work on that
client's diagnosis form will proceed until the client has rectified the problem,
or uses surface-mail.
Periodontal-net will not be held responsible for diagnosis forms and consultant's
advice sent by e-mail whose systems or word-processors are not compatible.
All diagnosis forms and reports are produced in Microsoft Office Word format.
Periodontal-net will not be held responsible for diagnosis forms or reports
being lost or damaged in the post.
All fees are charged as advertised on the Periodontal-net website.
All fees are payable in advance.
Once the client has placed an order using our secure order form and credit
card payment facility, it cannot be cancelled.
Periodontal-net reserves the right to refuse to carry out a client's instructions,
if the client offers false or inaccurate information for inclusion in their
diagnosis form. In such circumstances, the client's advance payment will be
returned after deduction of an administration fee.
Periodontal-net will hold completed diagnostic forms and consultant's report
for a period of 2 years, unless instructed to delete them from the archive.
Periodontal-net will not release an diagnostic form and the consultant's report
or in full or in part to a third party except by the explicit instruction
of the client.
